INSTRUCTIONS FOR USING CCHMC SHARED RESOURCE FACILITIES

OSU Submission to CCHMC Shared Resource

NOTE: If shipment involves direct shipment of animals, separate arrangements must be made. Contact Lilith Reeves at CCHMC 513-636-34368 or lilith.reeves@cchmc.org

Contact the appropriate CCHMC Shared Resource to arrange for services and obtain pricing information. Contact information for the CCHMC Shared Resources is available our Web site under each specific Shared Resource.

Contact OSUCCC Purchasing to arrange for a Purchase Order for desired services.

Contact OSU Research Core Contact (Teri Hall, 614-293-6884) to arrange for delivery of materials to CCHMC. In most cases, arrangements will be made to have the courier pick up the materials directly from your location. Note: In order to ensure that your materials can be received at CCHMC, all deliveries must be arranged by 12 p.m. on the date of desired shipment for courier delivery.

The following should accompany your shipment to CCHMC:

• A completed CCHMC Shared Resource Requisition Form
• A copy of the Purchase Order
• The name and delivery address of the CCHMC Shared Resource with a contact name and phone number

Once your material has been sent to CCHMC, please fax (x33305), e-mail (teri.hall@osumc.edu) or use the CCC mail to submit a copy of the completed CCHMC Shared Resource Requisition to the OSU Research Core Contact.

In the event that you receive an invoice or any other billing information from the CCHMC Shared Resource, please forward materials to OSU Research Core Contact.
